| STEPS TO APPLYING |
| |
| In order to submit an application for one of our academy programs, you must do the following: |
| |
| 1) Go to the instruction box below and click on the Get Form button. Print the form, complete it, sign it and bring it to your child’s middle school guidance counselor or administrator. They will need to follow the instructions on the form to complete the middle school section of the application.

| 2) In the instruction box below, click the Application button and fill out the information on the application form. Submit the application electronically by following all of the instructions. |

| 3) Make a note of the date and time you selected for the admissions test. Someone will contact you to
if there is a conflict with your first choice to confirm your child’s testing session.
All Accuplacer Admission Exams take place at MCST, 400 East Main Street, Denville, NJ. |
