According to state law the school nurse, parent/guardian, school physician and substitute nurse are the only individuals who can administer prescription AND over the counter medication during school hours and during school sponsored activities. All medication must be brought to the school health office by the parent/guardian in the original container, appropriately labeled with the name of the student, medication, dosage, prescription number, date, and physician or health care provider.
To have medication administered to your child during the school day, both you and your child's doctor must complete and sign the MCVTS Authorization to Medicate Form. Please see below to download.
If your child would like to have Tylenol (Acetaminophen) or Advil (Ibuprofen) during the school day, both you and your child's doctor must complete and sign the Authorization to Medicate Acetaminophen/Ibuprofen form. Please see below to download.
