If your student currently receives accomodations under Section 504 and you'd like to continue a plan at MCST:

Please obtain a current doctor's note with written diagnosis, and complete below documents (Consent to Review Records & Parent Input Form).  Once all documents are obtained/complete, please scan and email all completed documents TOGETHER in one email to 504@mcvts.org.

**Documents must be dated within a year of September 2020 and should be signed by a medical doctor. If you do not have updated documentation, please make an appointment with your doctor asap to obtain documentation.
