Incoming Freshmen 2024-2025

If your student currently receives accommodations under Section 504 and you'd like to continue a plan at MCST:

Please submit the following to 504@mcvts.org:

-Current 8th grade 504 plan

-Request for Accommodations Packet (6 pages, completed and signed by doctor and parent)

• Includes Formal Request for Accommodations, Medical Provider Form, Release of Information Form, and Parent Input Form

-8th Grade Teacher Feedback, if applicable (2 minimum) (form below- editable electronically)
