Anaphylaxis Care Plans

STUDENTS WITH A MEDICAL HISTORY OF ANAPHYLAXIS :

 

Please complete the Anaphylaxis Action Packet ( which is downloadable below). This must be completed annually in order to participate in MCST clubs, sports, and attend school-sponsored field trips.

 

  • Food Allergy & Anaphylaxis Emergency Care Plan :We will accept your physician's care plan or we have provided a care plan to be filled out by your provider.

 

  • Morris County School of Technology Physician Certification for Self-Medication of Epinephrine

 

  • Epinephrine Auto-Injectors (if applicable)

 

  • Please verify your child's epi pen is not expired.
  • You must advise the transportation department of your child's home school district that your child has anaphylaxis.  It is the responsibility of your home school district to advise your child's bus driver that your child has anaphylaxis and to arrange for your child's bus driver to receive anaphylaxis training.
