Medications
According to state law the school nurse, parent/guardian, school physician and substitute nurse are the only individuals who can administer prescription AND over the counter medications during school hours and during school sponsored activities. All medication must be brought to the school health office by the parent/guardian in the original container, appropriately labeled with the name of the student, medication, dosage, prescription number, date, and physician or health care provider.
To have medication administered to your child during the school day, both you and your child's doctor must complete and sign the MCVTS Authorization to Medicate Form. Please see below to download.
If you would like your child to have access to Tylenol (Acetaminophen), Advil (Ibuprofen), or Tums during the school day, you must indicate your permission by checking the appropriate box in the Health Questionnaire on Genesis. This form must be completed and updated annually.
An order authorizing the administration of these medications has been issued by the school physician and may be carried out with documented parental consent. These medications are supplied by the Health Office.
- These forms will need to be renewed annually.
- Medications must be picked up at the end of the school year.
